Another injury that we see occurring in adolescents are growth plate injuries. These can be dangerous as it is usually traumatic from high impact events and oftentimes require surgical intervention to stabilize the fracture. As a result these structures change to become more dense, improve tensile properties, become more pliable, or work more efficiently. These are usually good stresses that allow our bodies to become stronger and more resilient, however in excess some loads or stresses can cause damage. I like to use the example of the long time powerlifter who only strength trained for years with little variation in training, and now has an arthritic back and knees from chronic excessive loads and wearing down of his joint surfaces. In contrast the long time yoga instructor who can fold up like a pretzel but now has severe nerve pain in her leg due to spinal instability and neural involvement. As an example when you use the knee joint, it is just as important to have adequate strength of surrounding musculature in order to stabilize the knee during movement and activity, as it is to have flexibility in that musculature to allow for proper movements to occur. The point being made here is that with repetitive training regimens that lack a variety of movements and/or appropriate loading, you can become too strong and lack mobility or too flexible and lack stability. Applying ice to the affected tissue will vasoconstrict the blood vessels which will decrease inflammation, numb sore tissues, acting as a local anesthetic, and slows down the pain messages being transmitted to the brain. That key factor is posture, and it can alter the length of muscles and create changes in spinal curvature, affecting the biomechanics of the cervical spine. The classic case of postural deficits creates what is referred to as "upper cross syndrome." The diagram used to depict this syndrome is an X drawn over the head, neck, and shoulders of an individual as you examine them from the side. Each axis indicates the muscles that are weak or lengthened and those that are tight or shortened. Commonly the muscles lengthened are the deep neck flexors underneath the chin and the scapular (shoulder blade) adductors or rhomboids and middle trapezius. In contrast, those that become tight are the pectorals (chest muscles) and the suboccipital group (small muscles at the base of your skull). These changes create that classic forward head and rounded shoulder appearance we see all too often. If you stretch the muscles that are tight and strengthening, those that are weak, you can alleviate your symptoms. However, repetitive activities or sustained postures likely create these postural abnormalities in the first place. Among the top of the list of activities to avoid is sitting at a desk for a long duration. Self-treatment strategies would include a doorway stretch targeting the pectorals and performing resisted rowing exercises targeting the scapula adductor group. In addition to focusing on those major muscle groups, chin tucks or head lifts would be ideal for helping address the deep intrinsic neck flexor tightnesses. Single Leg Squats With The Ball Stand sideways parallel to a wall with a stability ball between your left hip and the wall. Steps Downs With your right leg firmly on the step, bend the right knee with control, while dropping the left heel down towards the ground. Steps Ups Stand facing a step with your right firmly on the step. Wall Squats With Stability Ball Stand with your back to a wall, with a large stability ball between your back and the wall at the level of your low back. Wall Squats Stand 6-8 inches from a wall, with a small ball or towel roll gently squeezed between your knees. Seated Knee Extensions Sit on a firm chair with an ankle weight or resistance band around your ankle. Standing Hamstring Curls Bend your right knee up towards your buttocks, then lower with control. Sit To Stand Shift forward until your buttocks are near the front of the seat, and place your feet firmly on the floor with your toes under your knees. Hamstring Setting Sit or recline with your right knee bent to 45°, and your foot flat on the bed. Quads Setting Sit or recline with with your right leg extended in front of you and a rolled towel placed under your right knee.
